Either the number to be dialed has been entered incorrectly, or the area code you are dialing is incorrect.
Solution
Verify that the area code and phone number have been entered correctly.
Windows 95/98/NT Open My Computer and then Dial-Up Networking. Right-click the 95Net connection icon and select Properties. The phone number will be shown at the top. Windows 3.1 Trumpet Winsock must be running. At the top, click on Dialer and select Setup.cmd. Macintosh Generally, a telephone icon will appear in the upper right corner. Opening this icon should display the connection status. Edit the Properties of the 95Net connection.
As soon as my modem connects, I get an error message that I have been disconnected.

To verify it has been installed, open the Control Panel and double-click the Network icon. In the Configuration window, if TCP/IP is not installed, click the Add button. Select Protocol and click Add. Under Manufacturer in the left, click once on Microsoft and on the right double-click TCP/IP. You will need to restart the computer for the settings to take effect.

The quality and condition of your existing phone line prevents a higher access speed.
Solution
Modem connect speeds beyond 28.8K are highly dependent on the condition of your phone lines. 33.6K is actually rare ( 31,200 is more common ). The conditions have to be near perfect. A 56K connection has an even greater variance, ranging anywhere from 44K to 56K. If at all possible, try your connection from a different location to verify that it is indeed the phone line. You may also try visiting the modem manufacturer's web site for init strings and other items that may improve performance.Possibility
The modem's drivers were not installed correctly.

It's possible that a standard modem driver was used instead of the unique hardware drivers. The maximum speed you can attain with standard drivers is 28.8K. Use the software that came with the modem or visit the modem manufacturer's page for available drivers.
Possibility
Your modem is suffering from "Spiral Death Syndrome". This is a hardware problem. After connecting the modem drops speeds to compensate for any line noise. However, instead or adjusting the speed back up, it continues to lower the speed in a spiral. It continues to spiral to a point where the connection is either unstable or extremely slow.
Solution
A simple solution. Open My Computer and then Dial-Up Networking. Right-click the 95Net connection icon and select Properties. Click the Configure button, and then the Connection tab at the top. In the lower right corner, click on the Advanced button. At the bottom, in the Extra Settings box enter the following initialization string: S15=128 and then click OK.
How can I get the full 56K speed?

I signed up for a 56K account, but I can't get beyond a 33.6K connection.
Possibility
The modem being used is a K56Flex modem that has not been upgraded to the V.90 standard.
Solution
Open My Computer, and then Dial-Up Networking. Right-click the 95Net connection icon and go to its properties. At the bottom, you can view the current modem in use. Verify that the modem is either X2 or V.90 compatible. If it is not listed here, check the box and any corresponding documentation. If possible, upgrade the modem to the V.90 standard by contacting the manufacturer or visiting their website.Possibility
Your phone line is not capable of a 56K connection.
Solution

Is there any way to automate the dial-up connection, so I don't have to click Connect every time?

Solution
You can upgrade your browser to Microsoft's Internet Explorer 4.0. It uses a different Dial-Up Networking program that gives you the option to bypass the Connect To window.
